Badger Hospice, LLC is seeking a compassionate Hospice RN Case Manager to provide skilled, comfort-focused care to patients and families during the end-of-life journey. You'll coordinate care, manage symptoms, and collaborate with our interdisciplinary team to ensure every patient receives exceptional, personalized hospice care.

• 1 Full-Time RN Case Manager | Burlington, WI

What You'll Do

• Complete patient assessments and develop individualized care plans.
• Coordinate care with the interdisciplinary hospice team.
• Provide skilled nursing care, symptom management, and family education.
• Monitor patient needs and maintain accurate documentation.

What We're Looking For

• Current Wisconsin RN license in good standing.
• Hospice, palliative care, or case management experience preferred.
• Strong assessment, communication, and organizational skills.
• Valid driver's license, reliable vehicle, and ability to travel.
